To obtain medical records from Priority Urgent Care and Walk-In Medical Practice, please submit a formal request via fax to 860-926-4245. Please include a cover letter marked “Attention: Medical Records.” Attorneys and medical practices must also provide a signed HIPAA release from the patient. Note that additional documentation may be required depending on the nature of the request.

In accordance with Connecticut and Massachusetts regulations, a fee may be associated with the cost of copies depending on the purpose of the request. Our team will process all requests within 10 business days.
