Self-Pay rates:

• Basic Assessment Charge (Initial Visit) = $150.00
• Reassessment Charge (Follow Up Visit) = $125.00
• DOT = $150.00
• Telemedicine = $125.00
• COVID testing for Travel = $125.00
• Provider assessment with an X-ray or EKG: $200.00
• Provider assessment requiring sutures: $250.00
• Provider assessment requiring IV therapy: $300.00
• Provider assessment for a pre OPS physical with blood work and EKG: $200.00
• N95 Testing: $75.00
• EKG (No Provider assessment): $75.00
• In-house Drug Screen or Breath Alcohol Testing: $69.00

Physicals:

• Child, Camp, School, and Sports Physicals = $125.00
• Adult and Pre-employment Physicals = $150.00

• Patients Will Be Charged Additional Costs for X-rays, Blood Work, Ekg, Etc.

• Pre-op Physical (medically necessary) = Patient’s insurance is billed
• Pre-op Physicals (not medically necessary)

• Basic Pre-op physical = $125.00
• With labs = $150.00
• With labs and EKG = $200.00

• HUSKY Insurance shall pay self-pay rates.
